Engaging in the therapeutic process requires a commitment from both the therapist AND the client. Treatment facilitators may terminate services after 2 consecutive absences without notice as to why the absences occurred. For scheduled absences, care can be suspended and initiated after the client’s return.

Cancellations must be made by telephone call, text message, or in person communication.
A “late cancellation” is an appointment that is not cancelled at least 24 hours before the scheduled appointment time.
A “missed/no show” appointment is an appointment that is not attended at the scheduled time where the patient has not notified the office of the absence prior to the appointment start time. An appointment will be considered missed/no show after 15 minutes past the scheduled appointment time.
Late cancellations and missed appointments will result in a $95 fee.

Counseling minor clients require consent from a parent or legal guardian authorized to provide this consent. If the child’s parents are not married, PMCC staff must determine if one or both parents must consent to the treatment of the minor client.
In cases in which there have been court orders regarding custody of the minor client, a copy of the custody decree will be required prior to the first session, and examination of the legal paperwork will occur to determine the person(s) legally able to consent to treatment. A copy of the court order will be kept in the client file. If a parent’s rights have not been terminated, they may be contacted to participate in counseling with or without the consent of the other parent.

Requesting Your Health Care Records
You have the right to request a copy of your mental health records.
To request your records, please submit a written request to your treating clinician or to the practice directly. Requests may be made via email, secure client portal, or in writing. Records will be provided in accordance with Texas law and applicable privacy regulations.
